DEPARTMENAL POSITIONS

Medicaid Service Coordinator - Monday-Friday = 35 hrs.

This position entails handling a caseload of 30+ people with developmental disabilities. Responsibilities include writing, coordinating, and monitoring Individual Service Plans; advocating for and assuring linkages with community support services that are consistent with an individual's needs and wants; Must have knowledge of social service, medical, and educational resources in Suffolk County. Site visits are required.

Bachelors Degree in a Health or Human Service Field and one year experience working with people with a developmental disability or a case management position with another population, or MSW, or Masters Degree in Psychology or Human Service Field.

CHILDREN’S SERVICES

1:1 Teacher Aide - Bohemia, NY, M-F, 8:15 AM - 2:45 PM

Attends to the physical needs of the children; required to carry, transfer and position students as needed for feeding, toileting, diaper changing and positioning in adaptive equipment which entails bending.

Graduate of an accredited high school GED, knowledge of general health, safety and sanitation policies; ability to work effectively and cooperatively

ADULT DAY SERVICES

Driver – Work Activities (Bohemia), Per Diem

Under direct supervision of the Workshop Supervisor/Production Supervisor, an employee in this class primarily operates a truck for the purpose of pick-up or delivery of various production or program materials. Use of a van or car could also be required for various assignments.  Assists in warehouse operations and provides supervision and instruction to program individuals we support as needed.

A High School Diploma or GED; Readiness to learn and utilize relevant agency computer applications; Candidate must possess a valid New York State Driver's License at the time of hire and for the duration of employment.  If the employee is to drive an Agency vehicle or his/her own vehicle on Agency business, he/she must adhere to the Agency Fleet Safety Driver's Policy/Procedures.

Community Integration Specialist - (Lakeland, Knick, Bohemia) M-F, 8am -3:30pm. 

Responsible for developing and supporting participants to attain their chosen outcomes and planned activities. An employee in this position will be called upon to drive the program van on a regular basis as part of community activities. The ideal candidate is energetic, responsible, organized and flexible.

Module Instructor - Westhampton Beach REACH | BLENDED, M-F 8am - 3:30pm = 35 hrs.

Training and supervising participants with Developmental Disabilities in classroom and workshop settings while implementing their outcomes and goals.

COMMUNITY RESIDENCES

Clerical Assistant – (IRA) Bohemia, Monday – Friday 8:30am-4:30pm = 35 hrs.

Under direct supervision of the Administrator, performs general clerical and receptionist duties as required to fulfill mandates of our programs. Work involves confidential and varied tasks; Performs related work as needed; must have working knowledge of Microsoft Word, Excel and Outlook.

High School diploma or equivalency required along with prior office experience; must possess Intermediate Microsoft Office skills; must have working knowledge (Intermediate level) of MS Word, Excel and Outlook. Business writing and comprehension skills required.

  • Dix Hills – Saturday & Sunday, 7:00 am – 6:00 pm = 20 hrs.
  • Greenport – Monday – Friday, 4:00 pm – 9:00 pm = 25 hrs.
  • Huntington – Friday & Saturday, 10:30 pm – 7:00 am = 16 hrs.
  • Oakdale – Friday, 6:00 pm – Sunday, 10:00 pm = 35 hrs. (Sleepover Required)
  • Old Field – Monday – Friday, 11:00 am – 8:00 pm = 40 hrs.
  • Old Field – Friday, 5:00 pm – 11:00 pm, Saturday & Sunday, 9:00 am – 10:00 pm = 30 hrs.
  • Southold – Saturday & Sunday, 9:00 am – 8:00 pm = 20 hrs.
  • Westhampton – Saturday & Sunday, 7:00 am – 6:00 pm = 20 hrs. 

Instruct developmentally disabled individuals residing in a group home in Activities of Daily Living; Specific skills include personal hygiene and grooming, banking/budgeting, travel, household tasks, cooking, nutrition, laundry, and decision making.  Additional duties may include planning and implementing recreational activities and administering medications.

INTERMEDIATE CARE FACILITIES (ICF)

Part Time RN Charge – Shoreham, NY, Saturday- Sunday 7:00AM – 5:30PM = 20 hrs. Bi-weekly

Responsibilities include but are not limited to: assisting RN supervisors with on-site supervision of the weekend ICF staff nurses to assure all medical/nursing needs are addressed, availability to assess medical problems and advise on appropriate action, medication administration, in-service training in the area of nursing and health related issues.

Candidate must possess a current, valid NYS Registered Nursing License and a minimum of 2 years nursing experience. Experience with Developmentally/Intellectually Disabled population preferred.

Nurse – Sunday – Monday 1:30PM-10PM = 32 hrs. Bi-weekly

Under the direction of the Supervising Nurse, the ICF Nurse provides the appropriate medical/nursing care required to meet the needs of the developmentally disabled population; performs work as requested; Review, coordinate, and implement medical care of each program individual.

NYS Registered Professional Nursing License, or NYS Licensed Practical Nurse License; Candidate must possess a valid New York State Driver's License at the time of hire and for the duration of employment.
