DEPARTMENTAL

Nursing Supervisor/Trainer

Supervises the Adult Day Services nurses to ensure that all program participants’ medical/nursing needs are being addressed during the program day; Develops and implements training programs on medical topics to meet Agency and regulatory requirements; provides orientation training to nurses; develops and maintains written policy/procedure for Agency nursing practices; performs related work as required.

Graduation from an accredited nursing program; license and current registration from the New York State Department of Education as a Registered Nurse; Five years experience working as a registered nurse; two years Supervisory experience, one year working with the developmentally disabled population and experience with OPWDD regulations preferred.

Medicaid Service Coordinator - Full Time, Monday-Friday, 35 hours/week

This position entails handling a caseload of 30+ people with developmental disabilities. Responsibilities include writing, coordinating, and monitoring Individual Service Plans; advocating for and assuring linkages with community support services that are consistent with an individual's needs and wants; Must have knowledge of social service, medical, and educational resources in Suffolk County. The successful candidate must be able to work independently, as well as being flexible in scheduling working hours to meet the needs of individuals. Site visits are required; a valid NYS driver's license and a car are necessary.

Bachelors Degree in a Health or Human Service Field and one year experience working with people with a developmental disability or a case management position with another population, or MSW, or Masters Degree in Psychology or Human Service Field.

Community Integration Specialist (KnickerbockerM-F, 8am -3:30pm

Responsible for developing and supporting participants to attain their chosen outcomes and planned activities. An employee in this position will be called upon to drive the program van on a regular basis as part of community activities. The ideal candidate is energetic, responsible, organized and flexible.

FLOWERFIELD GARDENS GREENHOUSE

Flowerfield Gardens Greenhouse Worker (Holtsville)

Performs horticultural tasks and assists customers in the sales area.  As requested, performs related work; Under the supervision of the Production Supervisor-Greenhouse Manager, an employee in this class is responsible for performing a variety of horticultural tasks and for assisting in the sales area. 

High school diploma or GED; Documentation of a current Mantoux tuberculosis test that is within 12 months of beginning employment and a second step PPD within three (3) weeks of beginning employment must be provided.

Instruct developmentally disabled individuals residing in a group home in Activities of Daily Living; Specific skills include personal hygiene and grooming, banking/budgeting, travel, household tasks, cooking, nutrition, laundry, and decision making.  Additional duties may include planning and implementing recreational activities and administering medications.
