DEPARTMENTAL POSITIONS
 
Medicaid Service Coordinator - Monday-Friday = 35 hours/week

This position entails handling a caseload of 30+ people with developmental disabilities. Responsibilities include writing, coordinating, and monitoring Individual Service Plans; advocating for and assuring linkages with community support services that are consistent with an individual's needs and wants; Must have knowledge of social service, medical, and educational resources in Suffolk County. The successful candidate must be flexible in scheduling working hours to meet the needs of individuals. Site visits are required; a valid NYS driver's license and a car are necessary.

Bachelors Degree in a Health or Human Service Field and one year experience working with people with a developmental disability or a case management position with another population, or MSW, or Masters Degree in Psychology or Human Service Field.

CHILDREN’S SERVICES

1:1 Teacher Aide - Bohemia, NY, M-F, 8:15 AM - 2:45 PM

The 1:1 Teacher Aide is responsible for the following duties: Attends to the physical needs of the children; adheres to the Health Safety and sanitation policies, maintains a clean, safe and hygienic classroom; assists the teacher in implementing the education program;  Required to carry, transfer and position students as needed for feeding, toileting, diaper changing and positioning in adaptive equipment which entails bending, lifting and moving the students necessitating the use of proper body mechanics and transfer techniques, provides bus duty and playground supervision.

Graduate of an accredited high school GED, knowledge of general health, safety and sanitation policies; ability to work effectively and cooperatively

Art Teacher – Bohemia, NY, PART-TIME Contractor (4 days/week)

Under the general supervision of the Assistant Director, Education, a member of this class must possess the following: Ability to teach children through the modality of art, Ability to create programs according to students needs:  emotional, physical and social, Capable of adapting her/his approach to meet the physical and cognitive needs of the students, Awareness of goals and total program of the organization.

Bachelor’s Degree in Art or art-related area (i.e., Special Education & Art Experience)

ADULT DAY SERVICES

Vocational Instructor - (Bohemia-Work Activities) – M-F, 8am-3:30pm | Per Diem

Under the supervision of the Program/Workshop Supervisor, an employee in this class is responsible for training and supervising individuals with Developmental Disabilities in a variety of vocational tasks while implementing their outcomes and goals; Provides ongoing supervision and training to an assigned group of individuals; Maintains production according to schedule and inform Supervisor whenever schedules cannot be met

Vocational Instructor – Cleaning Team – Knick, M-F 8am-3:30pm = 35 hrs.

Under the supervision of the Program/Workshop Supervisor, an employee in this class is responsible for training and supervising individuals with Developmental Disabilities in a variety of vocational cleaning tasks while implementing their outcomes and goals. 

A High School Diploma or GED; Readiness to learn and utilize relevant agency computer applications

Community Integration Specialist - (Lakeland, Knick, Bohemia) M-F, 8am -3:30pm. 

Responsible for developing and supporting participants to attain their chosen outcomes and planned activities. An employee in this position will be called upon to drive the program van on a regular basis as part of community activities. The ideal candidate is energetic, responsible, organized and flexible.

  • Southold – Saturday & Sunday, 9:00 am – 8:00 pm = 20 hrs.
  • Westhampton – Saturday & Sunday, 7:00 am – 6:00 pm = 20 hrs.
  • Westhampton – Monday, Tuesday, Thursday, Friday, 4:00 – 8:00 pm = 16 hrs.
  • Instruct developmentally disabled individuals residing in a group home in Activities of Daily Living; Specific skills include personal hygiene and grooming, banking/budgeting, travel, household tasks, cooking, nutrition, laundry, and decision making.  Additional duties may include planning and implementing recreational activities and administering medications.

INTERMEDIATE CARE FACILITIES (ICF)

Case Coordinator – Shoreham ICF M-F (Full-Time/Flexible Schedule Required)

Ensures that each individual’s active treatment program is integrated, coordinated and monitored; performs related work as requested. Under the supervision of the Assistant ICF Director, an employee of this class is responsible for the direct observation of individuals, the review of data and progress, and the revision of programs based on individual need and performance.

A Bachelors Degree in a health related field and one year’s experience working directly with individuals with developmental disabilities. Candidate must be able to work a flexible 40-hour work week schedule, including some evenings and weekends.
